print the following documents and bring them (completed) to your first
appointment.

My office is in the First West Building, on the corner of 1st Ave. West and John St. in Suite #400, designated as Seattle Therapy Alliance
Have a seat in the waiting area, and I will be out to greet you at your scheduled appointment time.
Parking:
There is ample street parking on the blocks surrounding the First West Building, including free one-hour parking on John Street and $2.00/hour parking on 1st Ave. West. If you are driving to your appointment, you may want to come prepared with at least $2.00 each week in order to cover the cost of parking if the free spots are not accessible.
Building Hours:
The building is locked for security purposes in the evening. If the front doors are locked when you arrive, please wait by the doors, and I will be down promptly to let you in.
